Here is a basic template for an employee appraisal:


1. Employee Information:

  • Employee Name

  • Department/Team

  • Position/Role

  • Appraisal Period

2. Goals and Objectives:

  • Goals/Objectives set for the appraisal period

  • Progress made towards achieving the goals/objectives

  • Any challenges faced in achieving the goals/objectives

  • Any additional goals/objectives for the next appraisal period

3. Job Performance:

  • Review of job performance in the appraisal period

  • Key achievements in the appraisal period

  • Areas of improvement identified

  • Any specific job-related training required

  • Any feedback received from peers/clients

4. Competencies:

  • Review of competencies exhibited by the employee

  • Areas of strength and development needs identified

  • Specific actions recommended to develop the identified competencies

  • Any feedback received from peers/clients

5. Overall Performance:

  • Overall assessment of the employee's performance in the appraisal period

  • Strengths and areas of improvement

  • Recommendations for future development

  • Any additional comments or feedback

6. Goal Setting:

  • Specific goals/objectives for the next appraisal period

  • Actions required to achieve the goals/objectives

  • Review frequency for the next appraisal period

7. Signatures:

  • Employee's signature indicating review of the appraisal

  • Manager's signature indicating review and agreement with the appraisal
